PA LICENSING PROCESS

  1. Apply for your Mblex exam at www.FSMTB.org
  2. Email us at dmzadie@philyschoolofmassage.com with TRANSCRIPT REQUEST in the subject line to request a copy of your transcript be sent to FSMTB.
  3. Pass the MBLEX exam!
  4. After passing your Mblex, FSMTB will notify the PA Board of Massage Therapy.
  5. You may apply for your license before taking the test at CLICK HERE
  6. Be sure to read all questions and your submissions carefully.
  7. You will need to do a Mandated Abuse Reporter course. This may be done online and there are free options available such as www.reportabusepa.pitt.edu
  1. A hands-on CPR certification is required. You may not substitute an online course. The American Red Cross and American Heart Association both offer local classes.
  2. A criminal background check is required from the state police or FBI. There is a small fee for this report. Request a criminal report from PA.GOV - Click Here for PA.GOV
  3. Complete a Self-Query from the National Practitioner Data Base. Click Here for NPDB.GOV You will provide personal info and details about any other healthcare licenses you have held, and verify your identity electronically. You will receive full instructions online.When you receive the “Response to your Self Query,” you will need to upload it to your online application. The report will need to be uploaded, where prompted, in order to submit your application.
  4. Once your license application has been submitted with all required supplements, you will wait on the PA Massage Board to review and approve. This can take several weeks, as the volunteer board members work independently, and at their own paces.You can check your status online, but will receive notification and your printed license in the mail once you are approved.
  5. The state board will email you a form called a Verification of Education. You will email it to the school at dmzadie@phillyschoolofmassage.com. It will be completed and mailed along with your transcripts to the state board. This will be the final piece of the process.
